COVID-19 VACCINATION REGISTRATION – MEMBERS 12 YEARS AND OLDER
All eligible beneficiaries should register on the Electronic Vaccine Data System (EVDS).
To register, please click on the link below to access the EVDS Registration Portal.
Answer “no” to the question: “Are you a Healthcare Worker?”
You will be directed to the portal that asks: “Are you 12 years and older?” – Answer “yes”.
Proceed with registration by completing your details.
